Hi. I'm desperate, and I need any advice I can get. My dad (age 55) was diagnosed with orthostatic hypotension about 1 year ago, and it seems to be progressively getting worse. He is to the point now where he is unable to even walk around the house, to the point where we've gotten him a wheelchair just to be able to go from his bedroom to the bathroom, for example. This is after he has passed out and fallen multiple times, he's broke an arm, 6 ribs, 3 vertebrae in his spine, all on different occasions of falling, due to going unconscious while trying to walk. His blood pressure while standing has been recorded as low as 36/29, while his blood pressure when he is laying down is consistently almost a textbook perfect 120/80. This change happens within seconds of changing from a laying to standing position, with his sitting blood pressure also being very low, but not as low as standing. He is currently taking Midodrine (10mg 3 times a day), and was previously taking Fludrocortisone, however it caused significant hypokalemia for him, so his doctor had him stop taking it. Neither of these medications have really improved his condition in any significant way, and it just seems to be getting worse as time goes on. There are occasions now where he is going unconscious while in a sitting position, not even standing. He is also incredibly fatigued almost everyday, and usually sleeps for the majority of the day. Here is some of his medical history that I feel might be relevant: He had uncontrolled type 2 diabetes for many years due to not being able to afford medication or insurance, however it is under control now as of the last year and and a half. Secondly, since about 2016 he has been progressively losing weight without trying, or having any change in his diet. He is 6'1" and his starting weight was roughly 210lbs, and currently he is roughly 145lbs. His doctors have attributed this to his many years of uncontrolled diabetes, however they are not 100% certain. Thirdly, he has been taking a antidepressant to treat anxiety, called Venlafaxine (75mg once a day) for roughly 15 years with no side effects, however I have read that Venlafaxine can potentially have a side effect of orthostatic hypotension, particularly in older/geriatric age adults. Once again, he is 55 years old, so not geriatric age, but not young either. We are kind of at a standstill, and don't know what to do next. Thank you for reading, If anyone has any questions, I will try to answer to the best of my ability, and I would greatly appreciate any help or advice whatsoever, please and thank you.
